Job Description:
The inpatient Palliative Care team at TJUH cares for patients
with serious illness providing symptom management and
identification of goal directed therapy. Our interprofessional team
is made up of physicians, CRNP s, social work and spiritual care.
The requirements of the position are as follows:
- Works in collaboration with interdisciplinary team including
physicians, nurses, social worker and chaplain to provide
comprehensive, quality palliative care to patients and families,
including assistance with complex pain and symptom management and
complex medical decision-making.
- Collaborate and maintain effective working relationships with
other health care team members, including primary medical teams and
other consultants to develop holistic and realistic care plans
based on patient and family goals.
- Communicate effectively with patients and families for
understanding of diagnosis, prognosis and treatment plans.
- Complete accurate and timely documentation in patient
record.
- Supervise nurse practitioner students on the Palliative Care
service.
- Attend and participate in department team meetings.
- Participate in palliative care education initiatives throughout
the Jefferson enterprise. How many patients do they see? How many
days a week?
- M-F 8am-5pm
- Inpatient consultations locations: TJUH, JHN, Methodist, Logan
Square
- Daily patient load varies from 1-3 new consults and 1-4
follow-up visits
- On-call responsibilities approximately 6 weeks/year. Phone
coverage only, no in person coverage required. Role requirements:
- Graduate of an accredited school of nursing and nurse
practitioner program/physician assistant program with a Master s
degree. Acute Care Certification required for Nurse
Practitioners.
- Current license in good standing to practice in the state of
PA
- Minimum of two years RN experience, palliative care/hospice
experience a plus
- ACHPN certification a plus, should be able to obtain within 1
year of hire.
- DEA and prescriptive authority license
- Participation in scholarly work including QI, education,
research. Coverage pool:
- Any clinician including other APP s or MD/DO.
Metrics/Productivity:
- New patient consults and follow up patient visits
- Involvement in QI, research, education
