TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Bronx, New York, 10457 About The Position Weekly Pay Range - $2842 To $3158 Job Title: Registered Nurse (RN) Job Specialty: Case Management Job Duration: 13 weeks Shift: Day - 7.5x5, 08:00 AM - 04:30 PM Guaranteed Hours: 37.5 hours per week Experience: Minimum 1 year of inpatient case management experience. Critical care experience preferred. License: Active RN license required Certifications: Basic Life Support (BLS) required. PRI & SCREEN certifications preferred. Must-Have: - Knowledge of discharge planning and making referrals. - Ability to function independently. - Good critical thinking and time management skills. - Prefer experience with CarePort, MCG, and/or InterQual. - Prefer experience in Utilization Review. Job Description: - Participate collaboratively with an interdisciplinary team and patients for discharge planning. - Work on Med-Surg or Tele units and share responsibilities with a social worker. - Expected to manage a caseload of 17-25 patients, with the ability to handle at least 20 patients. - Expected to round on the unit with physicians/team. - Handle discharge to home, community, or skilled nursing facilities. - Provide assistance with obtaining housing, shelter packets for homelessness, hemodialysis packages, wound care referrals, and more. - Utilize an electronic medical record system for documentation. - Participate in 2 days of hospital orientation and 1-2 weeks with a preceptor. - Occasionally required to cover weekends or holidays with manager approval. - Adhere to dress code: business casual or any color scrubs except royal blue or maroon. - Participate in daily operations with a positive attitude. 25893099EXPTEMP Job Requirements Required for Onboarding ACLS - AHA BLS - AHA Core Competency Pharmacology Skills Checklist Specialty Exam TNCC 5 Benefits of Travel Nursing Constantly adapting to new environments and healthcare facilities challenges you to expand your knowledge and skills.
