Job Description:
TravelNurseSource is working with Cynet Health to find a
qualified Case Manager RN in Bronx, New York, 10457 About The
Position Job Title: Registered Nurse (RN) Job Specialty: Case
Management (Utilization Review) Job Duration: 13 weeks Shift: 8H
Days Guaranteed Hours: 37.5 hours per week Experience: Minimum 1
year of inpatient case management experience License: Required
Certifications: BLS, PRI (Patient Review Instrument) Certification,
SCREEN Training Certification Must-Have: - Prior critical care
experience preferred - Some knowledge of CarePort, MCG, and/or
InterQual preferred - Utilization Review experience preferred -
Ability to handle at least 20 patients Job Description: - Expected
to have knowledge of discharge planning and making referrals. -
Will work on a Medical-Surgical or Telemetry unit, splitting the
floor with a Social Worker. - Must function independently and work
collaboratively with an interdisciplinary team and patients for
discharge planning. - Expected to round on the unit with
physicians/team. - Positive attitude, critical thinking, and good
time management skills are essential. - Utilization Review is a
separate department; they handle insurance approvals and arrange
transportation. - Caseload typically consists of 17-25 patients; on
pilot floors, 12-17 patients. - Involves discharge to home,
community, or skilled nursing facilities (SNF). - Tasks include
obtaining housing, preparing shelter packets for homelessness,
handling hemodialysis packets, and making KCI referrals for wound
vacs. - A binder is provided as a resource. - Primarily covers
inpatient areas. - Scheduling follows a Monday to Friday, 8 AM to
4:30 PM shift (7.5-hour shifts with a 1-hour lunch). - Occasionally
may require weekend or holiday work with manager approval;
caseloads differ on weekends. - Dress code is business casual or
scrubs (any color except Royal Blue or Maroon; neutral colors
preferred); lab coat provided. - Department-specific orientation
includes 2 days of hospital orientation for EMR training and 1-2
