Nurse Practitioner Salary Range: $180,000 - $190,000 + Annual Incentive Program Requirements to apply:Degree: Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.Licensure: Licensed to practice as a Nurse Practitioner in NY required.Certifications: DEA Certification required and AANP or ANCC Board Certification requiredExperience: Minimum of one (2) year of experience working with a frail or elderly population or, if the individual has less than one (2) year of experience but meets all other requirements, must receive appropriate training from the organization on working with a frail or elderly population upon hiring. Minimum of two (2+) years in clinical nursing practice in-home care, care management, or related field required. Bilingual preferredSchedule:Monday Friday 8:00 AM 5:00 PMWe are seeking a full-time Nurse Practitioner to join our Bronx Mobile Program!Job OverviewThe Nurse Practitioner Mobile Program will work in collaboration with our Physicians, and community primary care physicians to provide quality primary care for older adults/ geriatrics. The Nurse Practitioner will support the participant s functional, clinical, and psychosocial needs, with an emphasis on independence and wellness. This role will work as the liaison between the participant, the community physician, and plan staff to develop and implement the plan of care including person-center goal development, communicating changes in participant condition, and facilitating primary care preventative services.Benefits
