Remote RN Case Manager - CA Licensed

Job Description:

A company is looking for a Case Manager RN to provide telephonic case management and support for a diverse member population.



Key Responsibilities

Make outbound calls to assess members' health status and identify treatment barriers
Coordinate care and make referrals to outside sources as needed
Educate members on disease processes and encourage healthy lifestyle changes


Required Qualifications

Current unrestricted Multiple state RN licensure - Compact License
3+ years of experience as a Registered Nurse in a hospital or acute care setting
Ability to navigate a Windows environment and utilize Microsoft Word and Outlook
Private home office and access to high-speed Internet
Unrestricted California RN license with multi-state compact licensure preferred
