  • Full-time Nurse Practitioner position at Mercy LIFE - West Philadelphia, a member of Trinity Health PACE.
  • Provide primary medical management in collaboration with the PACE Organization (PO) Physician(s).
  • Perform assessments, develop and implement plans of care, evaluate participant complaints, and provide education.
    • Job Requirements
    • ANCC/AANP Board Certified - REQUIRED
    • Active PA license - REQUIRED
    • 5-7 years of NP experience required, preferably in long-term care, with a minimum of one (1) year working with the frail or elderly population - REQUIRED
    • BLS - REQUIRED
    • CDS Certification - REQUIRED
    • DEA Registration - REQUIRED
    • Certification as a Nurse Practitioner with prescriptive authority - REQUIRED
