Job Description:
A company is looking for a Bilingual Care Coordinator
(LPN/LVN).
Key Responsibilities:
Conduct monthly care coordination phone calls with patients and
healthcare providers
Create and revise personalized care plans in collaboration with
healthcare practices
Identify social determinants of health and coordinate referrals to
address gaps in care
Required Qualifications:
Active accreditation as an LPN with at least 1 year of
experience
Experience with multiple EMR/EHR systems and Microsoft
Office/G-Suite
Ability to work independently in a remote environment
Experience serving a geriatric population is a plus
Case management or care coordination experience is a plus
